Enroll your hospital

If you would like to enroll your hospital in our Hospital Access Patient Assistance Program, please download the application or just give us a call at 1-800-652-6227 to request an application. This program operates using a product replacement model. Hospitals are eligible to participate if they participate in the 340B drug pricing program and are classified as a disproportionate share hospital. This program is for outpatients only, and product must be dispensed from an outpatient pharmacy.

Hospitals that participate are required to sign a contract and agree to on-site assessments.
